Occupational Therapist

JOB SUMMARY:

Evaluates and treats patients referred by health care providers according to current applicable federal, state and local standards, guidelines and regulations.

QUALIFICATIONS:

  • Graduate of an accredited school in Occupational Therapy: Bachelors degree in Occupational Therapy required, Masters Degree preferred
  • Current Occupational Therapy license in the State of Nebraska
  • Basic Life Support (BLS)
  • Valid driver’s license in the State of Nebraska
  • American Occupational Therapy Association registration
  • Must be familiar with the principals and practices of Occupational Therapy according to the AOTA Code of Ethics and as outlined by the State of Nebraska Health and Human Services
  • One year previous experience preferred

ESSENTIAL JOB DUTIES & RESPONSIBILITIES

  1. Performs Occupational Therapy treatment procedures or delegates procedures to a Certified Occupational Therapist Assistant or therapy aide based on initial and ongoing client evaluation.
  • Evaluates clients by utilizing appropriate diagnostic testing of muscle function and activities of daily living.
  • Develops and implements the client’s plan of care in collaboration with the health care provider, other consulting health care professionals, family/caregiver according to agency procedures and state and federal regulations.
  • Documents the care provided, instructions given, clients progress toward goals, and changes in condition adhering to BCHC’s policies and procedures and state/federal regulations. Designs and implements an ongoing restorative program utilizing appropriate support personnel, i.e. family members, nursing staff, restorative aides, and Rehabilitation Aides.
  • Re-evaluates patient on a regular basis and adjust treatment accordingly using subjective and objective information.
  • Provides cost effective health service in the most appropriate setting as directed by the clients medical and social needs.
  • Designs and provides positioning device and staff instruction to prevent contracture deformities and to maintain optimal postural stability.
  • Integrates both manual and non-manual therapy skills in treatment of patients where indicated.

  1. Communicates effectively.
  • Attends and actively participates in client care conferences whenever on duty.
  • Follows approved shift report format when communicating patient information.
  • Identifies and recognizes abnormal symptoms/changes in patient condition, established priorities, and takes appropriate action.  Appropriately reports condition changes to medical provider.
  •  Communicates and collaborates effectively with other health team members regarding patient condition, patient satisfaction needs and recommendations for meeting identified outcomes.
  • Consults with the attending health care provider and obtains an order (when needed) for changes in the client’s plan of care.
  • Communicates effectively with client’s, families, staff and support services to achieve client goals and meet service expectations.
  • Teaches in-services relating to Rehabilitation upon request of supervisor and/or facility Director of Education.

3.   Promotes and ensures patient and staff safety in all aspects of responsibilities.

  • Administers treatments and procedures in a timely and safe manner according to health care provider orders including appropriate performance of standard precautions, using two patient identifiers and performing patient risk assessments every shift.
  • Utilizes proper body mechanics, transfer/lifting techniques to minimize fall risk to patient and injury to self.
  • Uses National Patient Safety Goals (for critical access hospitals) to ensure patient safety.
  • Implements infectious disease controls as appropriate including isolation procedures, proper use of hand washing and gloves.
  • Conducts safety assessments in client’s home.

4.  Plans and oversees activity programs for clients.

  • Plans programs involving activities, such as manual arts and crafts; practice in functional, prevocational, vocational, and homemaking skills, and activities of daily living.
  • Plans programs of participation in sensorimotor, educational, recreational, and social activities designed to help clients develop or regain physical or mental functioning or adjusting.
  • Selects constructive activities suited to client’s physical capacity, intelligence level, and interest to upgrade client to maximum independence, prepare client for return to employment, assist in restoration of functions, and aid in adjustment to disability.

  1. Prepares and uses necessary equipment.
  • Lays out materials for client’s use and cleans and repairs equipment at end of sessions.
  • Designs and constructs special equipment for individual and makes suggestions regarding adaptations of individual’s work‑living environment.
  • Inventories and requisitions adaptive equipment and all other supplies needed.
  • Utilizes space and equipment in cooperation with other staff.

  1. Evaluates treatment efficacy and patient progress and makes any necessary adjustments in the care plan to optimize patient care.
  • Educates family and/or support system to functional needs, equipment/resources to meet those needs.
  • Develops a discharge plan in an ethical and timely manner.

  1. Performs and maintains documentation.
  • Documents accurate and ongoing assessments of patient status that reflect therapy interventions, patient responses, patient teaching and status of outcomes at discharge.

  1. Performs other duties as assigned.
