What You’ll Do
As our Chronic Care Manager, you’ll serve as an advocate, educator, and care coordinator for patients with ongoing healthcare needs. Working closely with providers and an interdisciplinary healthcare team, you will:
- Coordinate care for patients with chronic and complex medical conditions.
- Develop individualized care plans based on patient assessments and goals.
- Connect patients with specialists, community resources, and support services.
- Monitor patient progress and adjust care plans as needs change.
- Help remove barriers to care while promoting continuity across the healthcare continuum.
- Educate patients on chronic disease management, medications, preventive care, and healthy lifestyle choices.
- Provide coaching and encouragement that empowers patients to actively manage their health.
- Maintain regular communication with patients through phone, electronic communication, and in-person interactions.
- Collaborate with physicians, advanced practice providers, nurses, and other healthcare professionals to optimize patient outcomes.
- Accurately document assessments, care plans, interventions, and patient communications within the electronic medical record.
- Participate in quality improvement initiatives and support evidence-based patient care.
What We’re Looking For
We’re seeking a compassionate healthcare professional who enjoys building relationships, solving problems, and helping patients succeed.
Qualified candidates will have:
- Current Nebraska RN or LPN license in good standing.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to effectively coordinate care among multiple providers and community resources.
- Strong organizational and documentation skills.
- Experience in chronic disease management, case management, care coordination, ambulatory care, or population health is preferred but not required.
- Experience using electronic medical records.
